Acknowledgement of Policies and Procedures
By signing this application, I hereby grant Ottley Music School permission to photograph, film, or otherwise record my child while participating in school activities, events, or programs. I authorize the school to use these images and/or recordings in print, digital, and online publications, including but not limited to the school website, social media, newsletters, and promotional materials, without compensation.

This permission is granted for the current school year and may be withdrawn at any time by written notice to the school.
