Heartworm testing is recommended annually to detect disease in its early stages so that treatment is less traumatic for dogs and to confirm that the medication is working as expected if the owner has been consistent in giving the medication.
I understand that the veterinarians at Hilltop Animal Hospital recommend heartworm testing annually, with or without the usage of heartworm prevention year round. However, you may sign a waiver every other year to continue refilling your pet’s heartworm prevention provided that you have established compliance in giving heartworm preventative according to our hospital records. Heartworm testing is REQUIRED every other year.
Owners should be aware that the manufacturer’s guarantee for heartworm prevention for products only stands if the product is purchased from a veterinarian. Annual heartworm test and monthly compliance in giving the medication is required by the manufacturer for the guarantee to remain in effect. The guarantee will not stand if the medication is purchased at a third party or online pharmacy.
I have read and understand the risk involved and choose to decline heartworm testing at this time. I do hereby release Hilltop Animal Hospital is its doctors and staff from any liability should any disease result from my dog not being annually tested for heartworm disease.
