A major or significant Injury is likely to result in extended time out of football, any head injury/potential concussion and/or any injury likely to require further medical care/financial cost.
If the injury is minor and the player can return to play almost immediately without requiring further assessment, the detailed sections of the form do not need to be completed.

A major or significant injury may present with one or more of the following symptoms:
- Severe swelling or pain
- Visible lumps, bumps, or deformities
- A popping or crunching sensation in the joint
- Weakness or inability to bear weight
- Joint instability
