Please complete this form to apply for your student to attend Wayman Academy in 2025-2026.

A student who is currently enrolled in a private school, an out-of-county school, or who is entering the Duval County Public School system for the first time must complete the following information before the student’s application can be processed. If the student has an out-of-county address, the county in which the student resides as well as the attendance zoned school’s name is required in order for a student number to be created.
