blanks I give permission to the Club to seek emergency medical treatment for my child. If I cannot be reached, the Club has permission to have my child transported to the nearest medical facility. I understand that I will be responsible for any and all costs of medical attention and treatment. Further, I understand it is my responsibility to inform Club staff of any medical and/or dietary needs for my child. Doy permiso al Club para que busque tratamiento médico de emergencia para mi hijo. blanksI understand that my child may not distribute medication of any kind (over-the-counter or prescription) to other members during Club time. I also understand that Club staff may not provide or administer any medications (over-the-counter or prescription)to members. blanks I give permission for my child to access the internet at the Club in accordance with Club policies. I understand that if my child does not follow Club policies regarding computer and internet use, Boys & Girls Club is not responsible for the consequences of such access (i.e. websites with inappropriate content). blanksI hereby understand, acknowledge, and agree that I am voluntarily choosing to enter my child into the Club. I further understand, acknowledge and agree that doing so, at this time, may present greater than normal risk due to the potential presence of the COVID-19 (Novel Coronavirus) among other Club participants and/or personnel, which virus is extremely contagious. I hereby acknowledge and agree that I am assuming any and all such risk, on behalf of myself, my minor child/children, and any and all of my family members and contacts. I acknowledge and agree that the Club is taking reasonable, preventative action to prevent the spread of any and all such illness among Club participants and personnel, but further, acknowledge and agree that there is no way to absolutely prevent the spread of same even with the taking of reasonable action. Accordingly, I hereby waive any and all right, and release the Club from any and all claims, that I have or may have in the future to pursue any legal and/or equitable claim against the Club which is directly or indirectly related to same. Por la presente entiendo, reconozco y acepto que voluntariamente elijo inscribir a mi hijo en el Club. Además entiendo,
reconocer y aceptar que hacerlo, en este momento, puede presentar un riesgo mayor de lo normal debido a la posible presencia del COVID-19 (Coronavirus) entre otros participantes y / o personal del Club, virus que es extremadamente contagioso. Por la presente reconozco y acepto que soy asumiendo todos y cada uno de esos riesgos, en mi nombre, en mi / mis hijos menores y en todos y cada uno de mis familiares y contactos. Reconozco y acuerda que el Club está tomando medidas preventivas razonables para prevenir la propagación de todas y cada una de esas enfermedades entre los participantes y el personal del Club, pero además reconozca y esté de acuerdo en que no hay forma de prevenir absolutamente la propagación de la misma incluso con la adopción de medidas razonables. En consecuencia, por la presente renuncio a todos y cada uno de los derechos, y libero al Club de todos y cada uno de los reclamos que tenga o pueda tener en el futuro para perseguir cualquier y/o reclamo equitativo contra el Club que esté directa o indirectamente relacionado con el mismo.
