Volunteer Enhanced DBS Application Process:
A minimum of one document is required from each group see the image below.
1. Group 1 Primary Identity Document
2. Group 2a Trusted Government Document
3. Group 2b Address & Social History Document
Once you have filled and submitted this form by making a payment of £25, an email will be sent to the MUS Office for processing.
You will then get an email with online portal login and password to carry on with further online form submission process
