  • Volunteer Enhanced DBS Application Process:

    1. Fill out this form making sure that the required information is clearly legible and pay the £25 processing fees.
    2. Once you have filled and submitted this form, an email will be sent to the MUS Office for processing 
    3. You will then get an email with online portal login and password to carry on with further online form submission process
    4. Once you finally submit all required information in the system, your documents will be checked and verified. If everything is fine, then an Enhanced DBS certificate is issued within 3~4 weeks

    1. Group 1 Primary Identity Document

    2. Group 2a Trusted Government  Document

    3. Group 2b Address & Social History Document
