  • Please initial below: Glohavenbeauty has certain policies in place to protect time that is set aside especially for you and make sure you are getting our full undivided attention, and of course the best lash work around town!
  • Deposits: All Bookings require a non-refundable deposit in order to lock in a booking appointment. All $30 deposits go toward your remaining balance and should be sent over to any of these options down below . Venmo:@GloHavenBeauty Rescheduling: I understand that things can happen out of your control,however if you need to reschedule your appointment please do so before 24 hrs ! No notice within the 24hrs will result to your deposit being forfeited & a new deposit will be required to continue with another booking (no excuses) Notice: please keep in mind I take pride in quality work and try to lash each and every lash as possible . With that being said I ask that you free up 1-3 hrs for your fills and Fullsets . Please note Rushing a set will only result to clumpy lashes and poor retention, be patient with your artist and trust the process the results will amaze you ! Extra: please respect my time and space. No extra people Arrival: Please arrive to your appointment on time with clean/makeup- free eyes and lashes. If you arrive late, it may not be possible to extend the time available for your booked service; if your service is shortened due to your late arrival, you will still be charged the full cost of the service. In some cases the appointment may have to be rescheduled within the first 24 hrs Cancellations/No Shows: Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in my day that could have been filled by another client. As such, I require 24 hours notice for any cancellations or changes to your appointment. Clients that provide less than 24 hours notice will be charged a 50% cancellation fee and no shows will be charged 100%. Refund policy: You are paying for artist time, product and other expenses used to provide you with a service. No refunds will be given for any reason on services or products. If you are unhappy with a service, you may contact us within 48 hours of your appointment to discuss your concerns and if a fix can be done to address your concerns, it will be done so with a complimentary 60 minute touch up or removal if it is at the fault of application or product. Any concerns addressed after 48 hours of your last appointment, or if you failed to follow the proper aftercare instructions, will be charged at full price for the service. Lash Fills: To be eligible for any fills you are required to book no later than 3 weeks ! 2 week (regalur fills) are no less than 60% retention. Please require a photo before booking 3week (fills) No less than 40% retention.Extra charges will be made if passed the three week mark anything less is considered a FULLSET if I can count the lashes you have left , it's too late for a fill. Please note that everybody's lash growth is different so book accordingly .    
  • Permission to Use Photos  (optional, if yes please initial below) I consent to “before and after” photographs for the purpose of documentation, potential advertising and promotional purposes.  
  • Waiver of Liability

  • I understand there are risks associated with having artificial eyelashes applied to and/or removed from my natural eyelashes. I understand that the eyelash extensions will be applied to the natural lash as determined by the technician so as not to create excessive weight on the natural eyelash thereby preserving the health, growth and natural look of the client’s natural eyelashes. I understand as part of the procedure eye irritation, eye pain, eye itching, discomfort and in rare cases eye infection may occur. I understand and agree that if I experience any of these issues with my lashes that I will contact my technician and have the eyelashes removed immediately and consult a physician at my own expense. I understand that even though the technician may apply and remove the eyelashes properly, that adhesive materials may become dislodged during or after the procedure, which may irritate my eyes or require further follow up care. I understand and agree to follow the after care instructions provided by my technician. Failure to follow the after care instructions can cause the eyelash extensions to fall out. I understand that in order to have the eyelash extensions applied to my eyelashes I will need to keep my eyes closed for duration of 2 hours or longer during the procedure. If I wear contacts, I may be asked to remove my contact lenses for the duration of the lash extension application or removal. I also understand that I will need to be lying in a reclined position. Any medical conditions that might be aggravated by lying still for a prolonged period of time may mean I will not be able to have the procedure performed on my eyes.  
  • This agreement will remain in effect for this procedure and all future procedures conducted by my lash artist. I understand that this agreement is binding and I have read and fully understand the information listed above. I also agree to defend, indemnify and hold harmless the eyelash extension artist from any and all claims, actions, expenses, damages and liabilities as a result of having this procedure performed.
