I have agreed to voluntarily participate in an exercise program, including, but not limited to, strength training, aerobic/cardiovascular exercise, and flexibility training, under the guidance of Team Testa Wellness/Fit Couple Colorado. I confirm that I am physically and mentally prepared to undergo a fitness program and I am cleared by a medical provider. I currently have no physical conditions that would be aggravated by a fitness program. 

I acknowledge that my enrollment and subsequent participation are purely voluntary and are in no way mandated. I understand and am aware that physical fitness activities, including the use of equipment, are potentially hazardous activities. I hereby release Team Testa Wellness/Fit Couple Colorado from any claims that may arise in the future as the result of injury or loss I suffer as the result of my participation in a fitness program. These conditions may include but are not limited to, heart attack, stroke, fainting, disorders in heartbeat, injuries to bones, ligaments, tendons, and joints, injuries to the back or neck, or any other illness or soreness that I may incur, including death. I hereby release Team Testa Wellness/Fit Couple Colorado from any claims, demands, and causes of action as a result of my voluntary participation and enrollment. I fully understand that I may injure myself as a result of my enrollment and subsequent participation in this program and I hereby release Team Testa Wellness/Fit Couple Colorado from any liability now or in the future for conditions that I may obtain.

 

Participation with a minor child:

I have full knowledge of the nature and extent of the minor child's risk inherent in the use of the facilities, equipment, classes, or programs and am voluntarily assuming all such risks. The minor's parent or legal guardian understands the minor's parent or legal guardian will be solely responsible for any loss or damage including death, minor sustains while using the facilities, equipment, classes, or programs, and by the Waiver and Release the minor's parent or guardian is

relieving Team Testa Wellness/Fit Couple Colorado from any and all liability for such loss, damage, or death. 

I understand that I am fully responsible for the actions of my minor child and assume all risks of illness, injury, and loss to the child and/or the child's personal property.

 

Photo Release:

Team Testa Wellness/Fit Couple Colorado periodically takes photographs and/or videos for advertising purposes including but not limited to print ads and website. I hereby grant Team Testa Wellness/Fit Couple Colorado permission to use my likeness (or my minor child) in a photograph in any and all of its publications, including website entries, without payment or any other consideration. I hereby irrevocably authorize Team Testa Wellness/Fit Couple Colorado to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing Team Testa Wellness/Fit Couple Colorado programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including a written or electronic copy, wherein my (or my minor child's) likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.

 

Cancellation policy:

All scheduled sessions, appointments, or meetings that are not canceled or rescheduled within 48 hours will result in a forfeit of the session, appointment, or meeting and a loss of the financial investment at the rate of one session. 

Clients arriving late will receive the remaining scheduled session, appointment, or meeting time unless other arrangements have been previously made.

 

I have read the above terms and understand this document to be a complete waiver and disclaimer in favor of Team Testa Wellness/Fit Couple Colorado.
