PLAYERS CODE of CONDUCT
As a player in Beast Basketball Training (BBT) programs, understand and agree to support the following Code of Conduct:
I agree to participate with a good attitude in all skill and knowledge exercises as assigned by the coach.
I agree to treat fellow players, opponents, coaches and referees with dignity and respect.
I agree to respect officials and accept referees' decisions without gesture or argument.
I agree to encourage teammates and refrain from being critical of their mistakes.
I agree to accept authority and supervision in a positive manner at all times.
I agree to communicate constructively with my coach, teammates and opponents.
I agree to refrain from foul language, taunting and trash talking.
I agree to exercise self-control at all times, setting a positive example for others to follow.
I agree to behave appropriately at all times and will conduct myself in a respectful and courteous manner at all times especially when involved in Beast Basketball Training events .
PARENT/GUARDIAN - CODE of CONDUCT
Beast Basketball Training (BBT) provides a safe and balanced learning environment for players of all abilities, that develops athletic skills, character, sportsmanship, and teamwork. Beast Basketball Training (BBT) wants to promote the physical, social and emotional development of youth participants. It is therefore essential for parents, coaches and officials to encourage youth athletes to embrace the values of good sportsmanship. Moreover, adults involved in youth sports events should be models of good sportsmanship and should lead by example by demonstrating fairness, respect and self-control.
As a parent or guardian of a (BBT) player, I therefore pledge to be responsible for my (and other any parent, guardian or guests of my child) words and actions while attending, coaching, officiating or participating in a youth sports event and shall conform my behavior to the following code of conduct:
I will remember that children participate to have fun and that the game is for the youth participants, not adults.
I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or other sporting event.
I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; verbal or physical threats; or using profane language or gestures.
I will not engage, or encourage my child or any other person to engage, in any behavior which would endanger the health, safety or well being of any coach, parent, player, participant, official or any other attendee.
I will treat, and insure my child treats, any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation, game play or ability.
I will not initiate, and will not tolerate my child initiating a verbal or physical fight, abuse, negative comments or scuffle with any coach, parent, player, participant, official or any other attendee.
I will not embarrass my child by calling attention to him/her through loud or rude behavior.
I will encourage my child to practice good sportsmanship.
I will never ridicule or yell at my child or other participant for making a mistake or losing a competition.
I will promote the emotional and physical well-being of all the athletes ahead of any personal desire I may have for my child to win.
I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game, and will take time to speak with coaches privately at an agreed upon time and place if I have an issue or concern.
I hereby agree that if I (or other parent, guardian, or guests of my child) fail to conform my conduct to the foregoing while attending, coaching, or participating in a (BBT )youth sports event I and/or my child will be subject to disciplinary action, including but not limited to the following in any order or combination:
Verbal warning issued by a league, (BBT) organization or an( BBT) official who is authorized to issue such warning.
Written warning issued by a league, (BBT) organization or an (BBT )official who is authorized to issue such warning.
Suspension or immediate ejection from an (BBT) event issued by a league, (BBT) organization or a (BBT) official who is authorized to issue such suspension or ejection.
Suspension from multiple BBT events issued by a league, (BBT) organization or a (BBT) official who is authorized to issue such suspension.
One season, multiple season or permanent suspension issued by a league, (BBT) organization or a (BBT) official who is authorized to issue such suspension.
Requirement to provide written and/or in person explanation of a failure to conform to the conduct outlined above with the (BBT) Staff.
I hereby agree that if my son/daughter is trying out for/playing on the 3 on 3 Beast Basketball Leagues, I have read and understand the information about the player assessment process, team selection process, coach selection process, player time commitment and minimum playing time that is posted I further acknowledge that this (BBT) Code of Conduct may be updated and modified on this form.
I have read, understood, and hereby agree to abide by the above Players and Parents code of conduct. I will also ensure my child will read and abide by the code.
I hereby grant permission to Beast Basketball Training (BBT), here after referred to as Beast Training to take and/or use photograph(s) and/or video(s) of any participant of Beast Training taken, in; publications, news releases, online, social media platforms and in other communications related to the mission of Beast Training.
I hereby grant permission to Beast Training to send me; emails, advertisements, news releases, and digital media content to my email address and/or phone number.
I understand that services will not be rendered until my invoice is paid in full. I understand all invoices will be sent to the parent/guardian email provided above.
1. I understand that all invoices that are paid in full, are NON-REFUNDABLE.
2. If you have a scheduled session, and need to cancel, you MUST do so within 12 hours of the session or it will result in a missed session.
3. If you are late to a session, you will not receive extra time. It is HIGHLY ENCOURAGED for the athlete to stay after and practice the skills learned.
5. I understand that there is an assumed risk and liability when training.
6. No call, no shows will result in a missed session, which is non-refundable.
I hereby acknowledge that I am the parent/legal guardian of the "Minor Participant" (Minor Child) above who is under the age of eighteen. In consideration for the benefits to be derived from My Minor Child’s participation in the basketball training, I, on behalf of myself and my Minor Child, hereby acknowledge the following:
(1) I am aware that all basketball activities involve risk;
(2) On behalf of myself and my Minor Child, I further understand and acknowledge that the risks of basketball training activities may result in injury.
Because of the danger of participating in the above activity I, both on my own behalf and on behalf of my Minor Child, acknowledge and understand the importance of following rules and regulations established by Beast Basketball Training Corp. My minor child hereby agrees to obey such rules, regulations, and instructions.
I further acknowledge that my Minor Child is in good physical condition and does not know of any condition or reason that my Minor Child should not participate in basketball training.
ON BEHALF OF MYSELF AND MY MINOR CHILD, I HEREBY RECOGNIZE AND ASSUME ALL THE RISKS ASSOCIATED WITH MY MINOR CHILD PARTICIPATING IN BASKETBALL TRAINING AND RELATED ACTIVITIES AND I AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS BEAST BASKETBALL TRAINING, DAESHAWN BEASLEY AND RAFFI BOYADJIAN, AGENT REPRESENTATIVES, AND VOLUNTEERS FROM ANY AND ALL OBLIGATIONS, LIABILITIES, CLAIMS, DEMANDS, COSTS, AND EXPENSES, INCLUDING ATTORNEY’S FEES, OR DEMANDS OF ANY KIND OF NATURE WHATSOEVER WHICH MAY ARISE OR IN CONNECTION WITH MY MINOR CHILD’S PARTICIPATION IN ANY ACTIVITIES RELATED TO BASKETBALL TRAINING, INCLUDING INJURIES SUSTAINED AS A RESULT OF THE SOLE, JOINT, OR CONCURRENT NEGLIGENCE, NEGLIGENCE PER SE, STATUTORY FAULT OR STRICT LIABILITY OF DAESHAWN BEASLEY AND RAFFI BOYADJIAN, BEAST BASKETBALL TRAINING CORPORATION.
The terms hereof serve as a release and assumption of risk for myself and on behalf of my Minor Child, as well as our respective heirs, estates, executors, administrators, and assignees.
In signing this agreement, I acknowledge and represent that I have read and understand it; that I am the parent or legal guardian of the named child; that I sign it voluntarily and for full and adequate consideration, fully intending to bind myself and the named child by the same; and that I am at least eighteen (18) years of age and fully competent.
