This form is for a tattoo booking inquiry for Dallas or Vaultt_tec. They are a Henderson, NV based tattoo artist at Tiger Tattoo. All pieces will be priced individually a price was pre-discussed or pre-priced (Flash). They do not tattoo persons that are under the age of 18. This form does not ensure booking. They will be contacting you as soon as possible, but if this is a time sensitive booking (you are traveling to the Las Vegas Valley and would like to get tattoo) please feel free to contact me via IG at Vaultt_tec or via email at Vaultt.tec@gmail.com. Do you have allergies to mangos, coconut oils, or do you have sensitive skin? (I use a specific ointment when I tattoo that may have the following ingredients, so in order to not provoke a adverse reaction I ask my clients of their allergies so I can use a different ointment.)

I understand that this form does not ensure a tattoo booking with Dallas. I understand that I will be contacted as soon as the artist is able to respond to my inquiry. I understand that I will be asked for a deposit, via e-transfer (Venmo, Zelle, or PayPal), to hold the appointment time with the artist. The deposit can range from $50 to $100 which does go toward your final tattoo cost. I acknowledge that until this deposit has been received my appointment has not been confirmed. I understand that this is a non-refundable deposit. If I need to cancel or reschedule my appointment I will do this no later than 24 hours prior to my appointment to avoid losing my deposit (for reschedule). I understand that if I make changes to my design after submitting this request and booking the appointment the cost may vary from the initial quote I was given. I understand that I will not receive a preview of my design prior to the appointment.
