    I, pet owner, do hereby entrust The Pet Resort to care for my pet while boarding during their stay and agree to all of the terms listed below. If I am unavailable to pick up my pet(s) on the previously listed date, I understand my pet may be moved to an alternate space if my originally booked space is no longer available. I authorize Lafayette Veterinary Care Center to do what they deem necessary for the health and well being of my pet and agree to pay for any and all expenses relating to same. I am aware that my pet, despite being vaccinated, could still be susceptible to contracting airborne illnesses such as kennel cough. My pet will be examined for fleas at check-in and treated if necessary at a charge of $10.00. I also agree to pay daily board at the rate agreed upon at the time of booking.

    *Please note that all oral or topical medications will be administered free of charge. Injections can be administered for a fee of $5.00 per injection.

    *Please note that Lafayette Veterinary Care Center provides food and food bowls for every pet. We serve Purina EN which is a prescription diet for sensitive tummies.

  • I agree that I am solely responsible for any damages that the above mentioned pet(s) may cause through malicious or improper conduct. I also agree to give permission for my pet(s) to have unrestricted and unsupervised access to my personal belongings listed on the front page during their stay at Lafayette Veterinary Care Center. Therefore agreeing to release Lafayette Veterinary Care Center from any liability of damages or harm that my pet may cause to my personal belongings or himself during their stay. 

    ***PLEASE INFORM US IF YOUR PET CAN CLIMB FENCES, WE NEED TO TAKE SPECIAL PRECAUTIONS TO PREVENT ESCAPE!

    I also understand that my puppy is not fully immune to communicable illnesses until 16 weeks of age with at least two sets of vaccines. Because of this, puppies 12-16 weeks old boarding at the resort are required to be housed in a front suite. 

    *We kindly ask that you limit belongings to 2 items per dog. Ex: 1 toy and 1 blanket

    I have read this agreement and understand the terms given from LVCC.
