Local 32, Office and Professional Employees International Union

Desiring to become a member of Local 32 chartered by the Office and Professional Employees International Union, affiliated with the A.F.L.-C.I.O., I hereby make application for admission to membership and authorize Local 32 to be my exclusive collective bargaining representative.

"I pledge my honor to faithfully comply with the Constitutions, Laws, and all amendments thereof, of Office and Professional Employees International Union, Local No. 32 and of the International Union. I further promise not to divulge or make public any of the private proceedings of this Union."

I authorize my employer to deduct from the wages due, or which may become due me the $40 initiation fee, and $39 monthly dues effective immediately following completion of my probationary period and to pay same to the Office and Professional Employees International Union, Local 32, A.F.L.-C.I.O.

This authorization shall be irrevocable for one year from the date hereof, or the expiration of the current union agreement or until removal from the bargaining unit, or until separation from employment, whichever is earliest, and shall be automatically renewable from year to year thereafter under the same conditions as above, unless a notice of cancellation is served in writing upon the employer and the union at least thirty days before the automatic renewable date. Union dues are not deductible as charitable contributions for federal income tax purposes. They may be deductible in some cases for business purposes.
