PLEASE NOTE: This withdrawal survey is not for students transferring between our schools
Informal "Non-Enrollment" Withdrawal:
We do not have the attendance to claim for this student. The student did not submit any attendance logs and no schoolwork has been completed.
Formal "Student/Family Choice" Withdrawal:
The student completed enrollment and completed curricular work, but they have been accepted into another school and/or has elected to enroll in a different schooling choice. Do Not Use for Non-Compliance. If you have a student who is Non-Compliance you must select Administrative Withdrawal and follow the Non-Compliance procedures found in the HST handbook.
Administrative Withdrawal - Non-Compliant:
The student attended your school and started the curriculum, but one of the following events occurred: Teacher is unable to contact the family, work has not been completed by the student, and/or student has multiple, unresolved absences. Must have approval and signature from a Director or Principal. Two, Non-Compliance letters must be uploaded into School Pathways.
Administrative Withdrawal - 5 Day Non-Residency Notification:
If there is reason to believe that a student’s residency is in question the school may investigate to determine the authenticity of the home address. When it is determined that a student lives outside of California and/or an unauthorized county, the school will provide written notice of the determination of non-residency within five days of the school’s intention to disenroll the student. The family is allowed to call a hearing within those 5 days to provide evidence to dispute the finding of non-residency. If the family does not request a hearing within the five days their right to a hearing is waived and the student will be immediately disenrolled. If the family initiates the hearing, the student shall remain enrolled and shall not be disenrolled until the school issues a final decision.
The student has completed all high school coursework, has enough credits, and has been cleared to graduate by the student's counselor.
Not Returning 21/22:
