  • Before submitting your application to withdraw, please take some time to review the Policy on Former PHRM Trainees Re-Entering the PHRM Training Program and discuss your situation with the PHRM Training Team.

    Please ensure that you print any copies of assessments prior to completing the online request form as once you are no longer a PHRM trainee these assessments are no longer accessible. To obtain these assessments once you have withdrawn, an administration fee will apply.

  • Responses to these questions are voluntary; however, all personal information will be deidentified and excluded from data analysis and reporting, with data reported only in the aggregate.

    We would appreciate it if you could provide feedback on your thoughts and experiences as a ACEM PHRM trainee, by answering the following questions. This feedback will assist us in identifying the strengths and weaknesses of the training program and to understand the issues experienced by trainees. This information will be used for quality assurance purposes, to inform and improve the trainee program and experience.

  • Finalising Your Withdrawal

  • 1. I understand that the withdrawal is effective from the date of the submission of this form to ACEM and no further training will be credited to my DipPHRM training record from this date.

    2. I understand that I will no longer be enrolled in the DipPHRM Training Program and will not have access to the ‘members only’ content on the ACEM website.

    3. I understand that any documentation requested from my trainee file post withdrawal will require payment of an administration fee.

    ACEM will notify you once your application to withdraw has been finalised.
