Guidelines and Instructions.

Please read carefully before continuing.

Please provide details as per the Passport/Birth Certificate only.

The official/legal name of the child will be used in any Certificates/Letters etc.

The following files must be ready with the following before you start:
1. Child's Birth Certificate Scan - PDF file of all sides of the certificate for Sri Lankans and foreigners.
2. Child's Passport Scan (if NOT Sri Lankan) - PDF file of the data page and valid Sri Lankan Residence Visa.
3. Child's Photograph - IMAGE file of the child's head and upper body.
4. Father's NIC Scan (if Sri Lankan) - PDF file of both sides.
5. Father's Passport Scan (if NOT Sri Lankan) - PDF file of the data page and valid Sri Lankan Residence Visa.
6. Mother's NIC Scan (if Sri Lankan) - PDF file of both sides.
7. Mother's Passport Scan (if NOT Sri Lankan) - PDF file of the data page and valid Sri Lankan Residence Visa.
Note: Camscanner app can be used for the scanning from smartphones. It is available for Android and Apple both.
