This form is to be filled out by a representative of the State 4-H office.

Each 4-H Member may only receive 3-Admission tickets and 2-One Day Parking Hangers.

Each Chaperone may only receive 1-Admission ticket and 1-One Day Parking Hanger.

We highly recommend that you complete this form prior to August 15, 2025!

Fullfillment of any requests that are received after that date may subject to delays, especially if you would like the tickets mailed to you.
