I, the undersigned, voluntarily agree to participate in a fitness program with Body For Days, which may include strength training, cardiovascular exercise, flexibility training, and other physical activities. I understand that participation in any fitness program carries inherent risks, including but not limited to muscle soreness, injury, or other health complications. I acknowledge that it is my responsibility to consult with a physician before beginning this program to ensure I am physically capable of participating. I assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation. I hereby release Body For Days, its trainers, and affiliates from any liability, claims, or demands arising from my participation in this program. By signing below, I confirm that I have read, understand, and agree to the terms of this consent form.
