    There are many drug and alcohol intervention programs out there that can help people conquer substance use disorders. However, at Community Arts Experience, Inc. (C.A.E.), we believe the key to helping increase resilience against substance abuse and other adverse outcomes amongst individuals requires education and caring connections at the community and family levels to cultivate and support overcoming hurts, habits, and hang-ups.

    TOURNAMENT OVERVIEW:

    Teams comprised of co-ed youth participants ages 7-17, parents, and adults from the Shenango Valley and surrounding faith communities will compete to win the coveted title of C.A.E.’s ‘Kick-The-Habit’ Kickball Community Champions and proudly house the 56” team trophy for one year.

    A complimentary lunch will be provided for all event attendees.

    During the lunch hour, participants will have an opportunity to hear a personal testimony from a special guest who was able to “kick the habit” of addiction and live a life of freedom.

    Additional educational resources will be made available throughout the event from C.A.E. STOMP partners and community services agencies. Cornhole game boards will be available to add to the fun for non-participating event attendees.

    • This is NOT a recreational program.
    • I assume all risks and hazards incidental to such participation.
    • I do hereby waive, absolve, indemnify, and agree to hold harmless the Community Arts Experience, Inc. – except to the extent and amount covered by our liability insurance.
    • I understand Community Arts Experience, Inc. will NOT provide medical insurance.
  • Informed Consent/General Release - Kickball Participants

    PLEASE READ CAREFULLY AND SIGN BELOW TO INDICATE YOUR AGREEMENT.
    NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY.

    Since participation in sports activities can be potentially dangerous, Community Arts Experience, Inc. requires that all youth and adult participants assume all risks associated with C.A.E.'s 'Kick-The-Habit' Kickball Tournament by signing this general release.

    For and in consideration of me and/or my child being permitted to participate in C.A.E.'s 'Kick-The-Habit' Kickball Tournament, I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which my child or I may have, or which may hereafter accrue to my child or me, as a result of my/our participation in C.A.E.'s 'Kick-The-Habit' Kickball Tournament during play or for any other reason during this event. This release is intended to discharge, in advance, Community Arts Experience, Inc., its officers, employees, community stakeholders, and volunteers, and the owners and maintainers of any indoor/outdoor facility used for C.A.E.'s 'Kick-The-Habit' Kickball Tournament from any and all liability arising out of or connected in any way with my or my child's participation in C.A.E.'s 'Kick-The-Habit' Kickball Tournament, even though that liability may arise out of negligence or carelessness on the part of Community Arts Experience, Inc., its officers, employees, community stakeholders, and volunteers, or the owners or maintainers of any indoor/outdoor facility used by Community Arts Experience, Inc. for C.A.E.'s 'Kick-The-Habit' Kickball Tournament.

    I further understand that serious accidents sometimes occur during sports activities and that participants occasionally sustain serious personal injuries, death, or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for me and/or my child to participate in C.A.E.'s 'Kick-The-Habit' Kickball Tournament and hereby agree to assume those risks to release and hold harmless Community Arts Experience, Inc., its officers, employees, community stakeholders, and volunteers, or the owners or maintainers of any indoor/outdoor facility used by the Community Arts Experience, Inc. for C.A.E.'s 'Kick-The-Habit' Kickball Tournament, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages.

    I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my and my child's heirs, executors, administrators, and assigns.

    I further agree to indemnify and to hold harmless Community Arts Experience, Inc., its officers, employees, community stakeholders, and volunteers, or the owners or maintainers of any indoor/outdoor facility used by the Community Arts Experience, Inc. for C.A.E.'s 'Kick-The-Habit' Kickball Tournament, for any loss, liability damage, cost or expense which may incur as a result of any injury or property damage me or my child may sustain while participating in the activity.

  • C.A.E.'s 'Kick-The-Habit' Team Registration

  • C.A.E.’s ‘Kick-The-Habit’ Tournament Rules:

    1. The kickball tournament is a double-elimination format. Each team is guaranteed to play a minimum of (2) games before being eliminated from the tournament. 
    2. Games are (4) innings or 45 minutes, whichever comes first.
    3. Each inning is comprised of (3) outs.
    4. A total of (3) strikes equals (1) out.
    5. Kicking the ball foul (4) times in one at-bat will be considered an out.
    6. Stealing bases is prohibited and will be considered an out.
    7. Leaving the base before the ball is kicked will result in the runner being called out.
    8. Players must tag up if a fly ball is caught before proceeding to advance bases.
    9. No headshots. Runners hit in the head with the ball will be safe except for sliding into a base.
    10. Limited to 15 players per team. Each team must consist of seven (7) co-ed youth between the ages of 7-17 and eight (8) adults. Ten players on the field at one time.
    11. The tournament will be held rain or shine.
  • EACH TEAM COACH/CONTACT IS RESPONSIBLE TO CONFIRM EACH PARTICIPANT LISTED BELOW HAVE READ THE INFORMED CONSENT/GENERAL RELEASE, FULLY UNDERSTAND ITS TERMS, AND AGREE TO GIVE UP SUBSTANTIAL RIGHTS.
