Shasta: Travel Disclaimer and Insurance Waiver

The undersigned individual desires to participate in the UHNLOCK:SHASTA retreat which is a 5 day retreat in Mount Shasta, California July 4th-8th 2024. UHNLOCK: Shasta has been organized by Meredith Sloane of Real Healing, Inc (“Promoter”). In consideration for the Promoter permitting me to participate in the UHNLOCK: Shasta retreat, I have agreed to execute this Release of Liability and Assumption of Risks (the “Release”).

I agree to release and discharge both Promoter and all of its employees, agents and representatives, as well as all other persons, corporations, or other entities that might have any liability to me (the “Released Parties”), from and against any and all damages, actions, claims and liabilities, whether known or unknown, anticipated or unanticipated, suspected or unsuspected, relating to or arising out of any activity, occurrence, or event involving the UHNLOCK:Shasta Retreat. This Release is intended to release and discharge the Released Parties from all damages, actions, claims and liabilities of any nature, specifically including, but not limited to, damages, actions, claims and liabilities arising from or related to the negligence of the Released Parties. I further agree to indemnify, hold harmless, and defend Promoter from and against any loss, damage, liability and expense, including costs and attorney’s fees, incurred by Promoter as a result of my participating in any activity sponsored by or involving Promoter in connection with the UHNLOCK:Shasta retreat.

The laws of the State of California shall govern the rights and obligations of the parties to this Release and the interpretation, construction, and enforceability thereof. I agree that any lawsuit brought against any Released Parties shall be brought solely in the California State courts sitting in Los Angeles, CA. Promoter reserves the right to use my name and any photographs of me alone or with others taken during the UHNLOCK: Shasta Retreat, in Promoter’s materials, brochures and website.

I HEREBY VOLUNTARILY WAIVE ANY RIGHT I MAY HAVE TO A TRIAL BY JURY IN ANY ACTION, PROCEEDING OR LITIGATION INVOLVING ANY RELEASED PARTY.

THIS RELEASE IS A BINDING LEGAL CONTRACT. PLEASE READ IT CAREFULLY BEFORE SIGNING. *Please print all of the required information legibly.
