Pets needs vary on a case by case basis, in which case additional charges may apply to the groom depending on the condition of the animals, age, weight, overall health, and temperaments. Do you consent to these additional charges should they be necessary for the care of your pet?*

I understand that the groomer Drake (Devon) Baker sets aside multiple hours for an appointment, and should there be no communication or notice of missing the appointment or arriving late after 15 minutes past appointment time, the appointment can be canceled at the groomers discretion.

I verify that I have read and understand the following statement
Grooming pets can be extremely stressful for them, and reactions can vary depending on age, grooming experience and medical conditions. You verify that you understand that a groom will be stopped immediately (no matter what step of the groom) if the pet is deemed too stressed to continue; to ensure the safety of both your pet, the groomer and his handler. You will not be charged should the groom be terminated as a result.*

I verify that I have read and understand the following statement
Anything can go wrong during a groom, even with every safety precaution taken. Devon (Drake) Baker and his handler prioritize the safety and health of every pet in their care. Heaven forbid a serious accident occur, you are aware that we will provide necessary first aid, and transportation to a vet clinic in cases of emergencies should the groom be terminated in this case, and not charged. Devon (Drake) Baker and his handler will not be liable for any veterenarian costs proceeding a groom.*

I verify that I have read and understand the following statement
Devon (Drake) Baker and his handler will not be held liable for clipper burn and/or minor nicks as a result of: matted and neglected coats, irritation caused by removing a coat from pets possessing mild to severe skin allergies, as well as minor tears in elderly pets with fragile skin. Devon (Drake) Baker and his handler are not responsible for the stressful effects that grooming may have on a geriatric pet. You are satisfied that Devon (Drake) Baker and his handler will prioritize preventing stress and injury at all costs.*

I verify that I have read and understand the following statement
Devon (Drake) Baker and his handler are not responsible for any allergic reactions resulting from the manufacturer's recommended usage of any products. Please inform us ahead of time of any possible allergies your pet may have, and we will do our due diligence to ensure we use the safest product for your pet. We recommend getting pets tested for allergies at age milestones to ensure the best quality of life.*

I verify I have read and understand the following statement
Devon (Drake) Baker and his handler are not responsible for pre-existing medical conditions or the aggravation of those conditions; such as, but not limited to: heart disease, arthritis, obesity, infections, or any other medical problem that may be affected by the grooming process.*

I verify that I have read and understand the following statement
Devon (Drake) Baker reserves the right to charge additional fees for services we consider over and above the what is covered by our standard rates. Devon (Drake) Baker reserves the right to refuse service to customers who pose a threat to our employees and other pets in our care, whether it be aggressive behaviour, health concern, or communicable parasite.
Should the owner's pet expose the groomer with parasites, there will be an additional $50.00 fee charged to cover Devon (Drake) Baker's expense for fumigation of the premises. It is necessary to fumigate to prevent contamination to other pets.
We will never de-mat an extensively matted (pelted) pet. De-matting is painful for the pet, can be dangerous depending on severity, and can have lasting impacts on anxiety and negative associations with the grooming process. Should your pet be pelted, we recommend completely shaving the coat and create a plan going forward, where the owner recognizes the importance of maintenance (brushing/combing) and regular appointments for their pet's health.
When a pelted pet is shaved down, pre-existing health conditions may be exposed. When a pet is pelted, airflow to the skin is disrupted leading to irritation and build up of bacteria. A shave down can uncover these skin conditions, as well as other problems that may have developed from the lack of airflow and maintenance.
Please note that it is difficult to shave down a pelted pet without slightly irritating the skin, since the groomer must remove them by shaving underneath the mats. THE ONLY WAY YOUR PET WILL STRIPPED DOWN WILL BE WITH YOUR CONSENT.
· I, the undersigned, owner or person representing the owner, do hereby release Devon (Drake) Baker and his handler from any and all responsibility resulting in stripping my pet and understand and support the conditions stated in this agreement.
Duty to Disclose- I have disclosed and will continue to disclose on an ongoing basis, any and all medical or other conditions, including but not limited to personality concerns or behaviors that may affect, limit or prevent my pet's ability to be cared for by Devon (Drake) Baker
I hereby agree to expressly and forever generally waive, discharge claims, indemnify, release liability, save, hold harmless and defend Devon (Drake) Baker and his handler, their invitees, sponsors, and advertisers (The Releases) from and against any and all injury, liability, claims, litigation, actions, suits, costs, losses, damages, attorney's fees, expenses or demands of every character whatsoever on account of, arising out of, resulting from or relating in any way to (1) any act or omission of the releases, including negligence, and (2) me or my pet's participation with Devon (Drake) Baker and his handler, or otherwise.
I agree that this release shall be binding on me and my successors, heirs, legal representatives, and assigns. I also expressly and forever release Devon (Drake) Baker and his handler from any duty to protect me or my pet(s) from injury of any kind, and agree that even if Devon (Drake) Baker chooses to implement safety precautions; such actions shall not alter the fact that I have released Devon (Drake) Baker and his handler from any duty to protect me or pet(s).

PLEASE READ CAREFULLY, THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS. IT DEPRIVES YOU OF THE RIGHT TO SUE DEVON (DRAKE) BAKER AND RELATED PARTIES. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY. SEEK THE ADVICE OF LEGAL COUNSEL IF YOU ARE UNSURE OF ITS EFFECT.
