I give permission for the above student to travel to Warren Dunes State Park, Sawyer, MI for BCHS Cross Country Camp, July 21 - 23, 2025 and traveling to and from the camp via BCHS Mini Buses. Recognizing the possibility of injury associated with any event and in consideration for Bishop Chatard High School (BCHS) and its affiliates who are accepting the registrant for these activities, I hereby release, discharge and otherwise indemnify BCHS, BCHS affiliated staff and sponsors, their employees and associated personnel, including the Archdiocese of Indianapolis against any claims by or on behalf of the registrant as a result of the registrant's participation.

I also authorize the BCHS personnel to act for me according to their best judgment in any situation that may require medical attention.

I give permission to BCHS to use photographs of my child taken during the camp activities, for BCHS marketing or other promotions.
