The named Guest(s) does hereby consent to the recording and distribution of reproduction(s) of the Guest's voice and performance as part of the media program entitled “Your Resource For Success Podcast or Your Resource For Success “360 & Me” Vlogcast Show” (herein referred to as the "Program"). This is to confirm that the undersigned has agreed to be interviewed or otherwise participate in the Program, a production of the Women’s Small Business Initiative, LLC (WSBI, LLC).

By signing this form, I hereby give permission to WSBI, LLC to use my personal image(s) in whatever medium deemed appropriate by WSBI, LLC for any of the following purposes: public relations, training and education. WSBI, LLC will not use the personal image(s) for any other purposes. I understand that my consent is voluntary and I release and discharge WSBI, LLC owner, employees, and each and all persons involved in creating my personal image(s) from any liability connected with the taking, recording, filming or publication of said interviews, photographs, slides, computer images, videotapes or voice recordings. I waive all rights I may have to claims for payment or royalties in connection with any exhibition, televising, internet posting, or other publication of my personal images, irrespective of whether a fee for its use is charged by any third party.

As a condition of publication/broadcast and for no monetary compensation, Women’s Small Business Initiative, LLC requests the non-exclusive worldwide rights to reproduce, distribute, and sell your oral and/or video presentation, in whole or in part, in any media, as part of this Program published under the auspices of Women’s Small Business Initiative, LLC and to license these rights to others, in perpetuity. Women’s Small Business Initiative, LLC shall have the right to edit and/or transcribe your presentation.

You also grant Women’s Small Business Initiative, LLC the right to use your name, biography and likeness in connection with the Program.

You warrant that your presentation is original with you, that publication/broadcast will not infringe on the rights of others, and that you have full power to grant this license. Should your presentation/broadcast incorporate copyrighted materials of others, you warrant that you have obtained such permission from those parties for publication/broadcast by Women’s Small Business Initiative, LLC, consistent with this Agreement.

If the foregoing terms are satisfactory, please sign and date this agreement below. Execution of this Agreement does not obligate Women’s Small Business Initiative, LLC to publish your presentation or other materials.

If the subject of the personal images is 18 years of age or older, the subject should sign below. I declare that I am eighteen (18) years old or older and am legally competent to execute this Consent and Release Form or that I have acquired the written consent of my parent or legal guardian. I understand that the terms herein are contractual and not a mere recital, and that this Consent and Release Form is legally binding on me.

I hereby consent to the use of my oral and/or video presentation/broadcast as described and agree with the provisions of this release form:

If the subject of the personal images is under 18 years old, the parent or legal guardian must sign below. By signing this Consent and Release Form, I consent to allow the minor named below to participate in the activities described above and allow the information obtained from the below named minor to be used and shared as described above.  I represent and warrant that I am eighteen (18) years of age or older and am legally competent to execute this Consent and Release Form under the laws of my country, I have the legal authority to represent the below named minor, I have read and understand this Consent and Release Form and I have had an opportunity to ask questions about the use of the below named minor’s personal Image(s), understand their purported use, knowingly consent to such use and voluntarily sign this Consent and Release Form on behalf of the below named minor.
