Due to the gradual re-opening from the pandemic and the transition in leadership, program structure and formats are still in the planning stages for the coming program year. We are optimistically confident, however, that we will be able to return to somewhat normal programming this year. I give permission for my child/children (named on this form) to participate in Children's and Youth Ministry activities at St. Thomas' Church, Garrison Forest. I grant permission to St. Thomas' Church, Garrison Forest employees and/or volunteer staff to make medical decisions with respect to said minor child/children in the event of accident or injury when parental consent shall be unavailable or when circumstance shall require immediate medical decisions, and to administer medication when required (every attempt will be made to contact parent/guardian).
MEDIA CONSENT: I give permission to St. Thomas' Church, Garrison Forest to use photographs; video recordings; audio recordings; and/or other visual/audio reproductions in which my child/children appear(s). I understand these materials are used to promote St. Thomas' Church, Garrison Forest's ministries. I understand that only my child's first name will be used if he/she is referenced.
