This Equipment Intake Form is just that. A form that notifies our team that you're sending or bringing equipment in and provides us with the necessary information to receive it when it arrives. Once you complete this form it will provide a PDF to download and print (a copy is emailed to you as well). Include the printed page with your equipment when you send or bring it in to us.
