I hereby give my consent for my child/ward to participate in the International Soccer Club of Fargo for the 2020-2021 season. Irecognize and acknowledge that participation in this program carries certain risks of physical injury, including permanent paralysis and even death. I agree to assume the full risk of any such injuries, damages or loss regardless of the severity which my child/ward may sustain as a result of participating in any activities associated with this program, including transportation to/from the activities. I recognize and acknowledge that participating in this program carries certain risk of catching Corvid-19. I hereby waive and relinquish all claims that I have against International Soccer Club of Fargo having read accept their "Returning to Play" policy. I hereby waive and relinquish all claims that I, my insurer, and/or my child/ward may have against the International Soccer Club of Fargo and its officers, volunteers and/or employees, and the Fargo Baptist Church and its officers, volunteers, and/or employees, from any and all claims from injuries, damages or loss which I and/or my child/ward may have or which accrue to me and/or my child/ward on account of my participation of my child/ward in this program.
