In furtherance of its mission and consistent with its values, Mid-Del Community Clinic endeavors to improve patient well-being while protecting the confidentiality and privacy of those we serve. All employees, medical staff members, students, volunteers and others who have access to patient or business information, are required to agree to the terms of this confidentiality agreement.
Please read this agreement carefully and ask questions if you need clarification.
“Confidential information” is any patient, physician, employee, or business information obtained during the course of work with Mid-Del Community Clinic.
All capitalized terms have the meaning set forth in our policies and the applicable law, including the Health Insurance Portability and Accountability Act (HIPAA).
I will treat all confidential information as strictly confidential, and will not reveal or discuss Confidential Information with anyone who does not have a legitimate medical and/or business reason to know the information. I understand that I am only permitted to access Confidential Information to the extent necessary for patient care and to perform my duties. I will not disclose identifying information (e.g. name, date of birth, etc.) if the information can be removed prior to disclosing or using the information.
I agree to use all Confidential Information only in accordance with clinic policies and procedures. I will not misuse or attempt to alter confidential information in any way.
I understand that violation of this agreement may subject me to no longer be able to work within this clinic.
I hereby acknowledge that I have read and understand this agreement and realize it is a condition of my access to the facilities of Mid-Del Community Clinic.
