If accepted into the volunteer program, I agree to work for The Animal Protectorates™ as a volunteer.
As a volunteer, I understand that I control the dates and times when I do the work and that The Animal Protectorates™is not responsible for scheduling my volunteer work. I also understand that I will not be compensated for any time spent volunteering, nor am I entitled to benefits, including employment insurance benefits upon termination of this agreement or as a result of this service.
I am aware that participation as a volunteer may have physical requirements (i.e. standing, lifting and carrying animals, crates, food, etc…) and will require the exercise of reasonable care to avoid injury. I am aware that animals that I come in contact with through my volunteer work may be sick, have contagious diseases and may be dangerous and cause injury to me. I am aware that participating in this type of volunteer work comes with the inherent dangers of being bitten and am voluntarily participating in these activities with knowledge of the hazards and potential dangers involved, and agree to accept any and all risks of personal injury and property damage.
The undersigned volunteer, in consideration of participating in any and all animal or other related activities, hereby forever releases and discharges The Animal Protectorates (TAPS) its volunteers, agents, independent contractors, employees, officers, directors, representatives, advisors, assigns, members, owners of premises (whether or not such premises is owned by TAPS), affiliated organizations and insurers and others acting on its behalf (hereinafter, collectively referred to as "Associates") in any location where activities are conducted and/or property are used from any and all claims, rights, demands, actions, causes of action, expenses and damages of any kind which he or she may ever have, whether known or unknown. The undersigned further understands the risk involved in participating in volunteer activities to include serious injury or death and fully assumes said risk for any injury, loss or damage of any kind resulting from such associated activities.
It is further understood that all volunteers should be current on immunizations and maintain a policy of insurance covering medical treatment and all related costs in the event of an injury as a result of participating in volunteer activities and that should said volunteer choose not to maintain a policy of insurance that they are liable for medical treatment and all related costs in the event of an injury as a result of participating in any and all volunteer related activities. Volunteers hereby agree to assume all expenses, medical, liability, or otherwise, arising out of any injury to them while participating in any volunteer related activity or event and understands that The Animal Protectorates ™ does not provide health, accident, or liability insurance. I UNDERSTAND THAT IF I AM INJURED IN THE COURSE OF VOLUNTEER WORK THAT I AM NOT COVERED BY THE ANIMAL PROTECTORATES WORKERS’ COMPENSATOIN PROGRAM. I authorize The Animal Protectorates™ to seek emergency medical treatment on my behalf in case of injury, accident or illness to me arising from my involvement as a volunteer. I understand that I will be responsible for medical costs incurred by such accident, illness or injury.
Waiver of Section 1542 of the Civil Code of the State of California
I have read this Release Agreement and fully understand that I will relinquish all claims or actions known now or in the future against TAPS, its board members, directors, officers, employees, agents, contractors, and volunteers. I am of legal age and legally competent to sign this agreement. I am signing this agreement of my own free will without the influence of a TAPS representative. It is expressly understood and agreed that I am hereby waiving and surrendering my rights pursuant to Section 1542 of the Civil Code of the State of California, which provides as follows:
"A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS WHICH A CREDITOR DOES NOT KNOW OR SUSPECT EXISTS IN HIS FAVOR AT THE TIME OF EXECUTING THE RELEASE, WHICH IF KNOWN BY HIM MUST HAVE MATERIALLY AFFECTED HIS SETTLEMENT WITH THE DEBTOR."
I hereby waive any and all provisions, rights and benefits conferred by any law or of any State or territory within the United States or any foreign country, or any principle of common law, which is similiar, comparable or equivalent in substance to Section 1542 of the California Civil Code.
I acknowledge that I must treat the animals and fellow volunteers with respect and dignity and under no circumstances will any form of abuse physical, verbal or otherwise be tolerated against any animal and person, and should said referenced events occur I will be asked to leave and will no longer be eligible to participate in any of The Animal Protectorates™ volunteer activities.
I understand that the materials and tools provided by The Animal Protectorates™ are and remain the property of The Animal Protectorates™, and I agree to return these tools and any remaining materials to The Animal Protectorates™ at the end of my volunteer service.
The person executing this release acknowledges that there is a valid consideration to
executing this release. The invalidity of any statement or waiver of rights above under local, state or federal law does not invalidate any other statement or waiver of rights above and have read, fully understand and voluntarily agree to this release and that no oral representatives, statements or inducements apart from this Release have been made to me.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY, AND SIGN IT OF MY OWN FREE WILL.

I have answered all questions on this form as truthfully as possible. I understand that if I have purposefully answered any of the questions untruthfully, I may be committing a felony and may be prosecuted to the fullest extent of the law.
