I understand that by completing this form, I am ready to move forward with the process of securing a childcare provider. The next step is to select my preferred site visit option: • Option 1: Virtual Site Visit - $100 • Option 2: In-Person Site Visit - $350 Once I select an option, I will submit the corresponding payment to the banking details provided. This payment confirms my commitment to proceeding with the service and becomes non-refundable once the Agency begins sending matched candidates. If no candidates are sent, the refundable portion will be returned to me (Virtual: $50 refunded, In-Person: $150 refunded). Turnaround Time: Matching is typically completed within 3-5 business days, depending on caregiver availability and my responsiveness. After payment is received, the Agency will schedule my virtual or in-person site visit. The fee must be paid before the site visit is conducted. Credits Applied When proceeding With Services for Full-Time Bookings (5 days-6 days per week or more)• If I choose the In-Person Site Visit ($350), I will receive a $250 credit applied to my first month’s invoice.• If I choose the Virtual Site Visit ($100), I will receive a $100 credit applied to my first month’s invoice.For Part-Time Bookings (3 days or less per week):• If I choose the Virtual Site Visit ($100), I will receive a $50 credit applied to my first week of service.• If I choose the In-Person Site Visit ($350), I will receive a $100 credit applied to my first week of service.
