I, as an applicant or employee, recognize that investigation services may be used byTrinity Bible College and Graduate School (TBCGS) to acquire my backgroundinformation. I further understand that TBCGS may, without any further notice beyond this authorization form, run periodic background checks, driver-record checks and credit reports on current employees as well as applicants. Such checks may take place at any interval of time as established by the Departmental head for each position.
I hereby release any organization, agency, or person from all liability for damages orclaims in responding to inquiries made by TBCGS. All persons, whether speaking onbehalf of themselves or on behalf of an organization or agency, may furnish any factual information concerning me including personal history, personal characteristics,achievement, performance, wage history, attendance, disciplinary information, and reason for termination of employment. Information regarding personal history and personal characteristics may include criminal history, driving history, and credit-related facts in addition to other pertinent information.
It is expressly understood that any information provided to TBCGS is to be used only for the purpose of determining my acceptability for employment. Once received by the college, personal information shall be considered confidential, stored in a secureenvironment, and viewed only by those persons directly involved in the decision-making process regarding my employment.
This authorization is valid throughout the hiring process, and, if I am employed byTBCGS, shall be valid throughout the term of my employment.
It is the policy of TBCGS to make such investigations and inquires as may be necessary for an employment decision. TBCGS policy states that information provided with job applications (including resumes), and the criminal history of an individual , may be investigated by the college for all positions.
Driving records are generally investigated only for positions that require the use of college vehicles, or when an individual requests that his/her name be placed on the list of authorized drivers. Credit histories are generally investigated only for a) Business Office positions where there is access to cash, bank accounts, or checking processing and b) Development positions where there is access to cash or confidential donor records.
