This form is to submit Partner's customer information which will include payment to Upstryve for those services. Once payment is received, a representative from Upstryve will reach out to your customer for further scheduling. I authorize the below charges to my card. I certify that I am the authorized user of the Credit Card that shall be submitted through this form. As long as the transactions correspond to the terms and conditions indicated in this authorization, I shall not raise disputes against the company.
All sales are final, and payments will not be refunded unless written confirmation from the Company. Furthermore, it is the Partners' responsibility to obtain the correct contact customer information. Any disputes arising from the Partner's customer, from conditions before services by Upstryve, shall remain the dispute with Partner and shall not be the responsibility of Upstryve.
