If possible, parents are advised to give medication at home and on a schedule other than school hours. Please check medication expiration dates before submission. If it is necessary that medication be given during school hours, the following regulations must be followed:
Medication must be brought to school in the original container appropriately labelled by the pharmacy or physician in English, stating the name of the student, the name of the medication, the time to be taken, and the amount of medication to be administered. Medication must be given to the school at the beginning of the school day. If the medication is NOT properly labelled, it will NOT be given.
Parent/guardian must sign this form, granting the school teacher or administrator permission to administer the medication, according to the regulations set.
我校建議家長儘量安排學生在校外時間服用有需要藥物。如學生有需要在上學時段服用藥物，家長必須在將藥物轉交校方前檢查其有效期，並須遵守以下規則：
帶回學校的藥物必須裝在藥房或醫生專用的容器中，並用英或中文貼上適當標籤，註明學生姓名、藥物名稱、服用時間、和服用劑量及次數。家長必須在早上課堂開始前向學校提供藥物。如果藥物標籤不正確，校方將不會允許學生服用。
