I understand it will be necessary for Generator's Maker Mentor Program as part of our volunteer background and investigation, to obtain consumer reports or prepare an investigative consumer report. The investigative consumer report may include, but not be limited to, reference checks, criminal history reports, social media checks and driving history records. Under the National Child Protection Act of 1993 Generator will conduct Criminal Records Checks, NSOPW checks, and the VT AHS Protection Registry Checks. Under the provisions of the Fair Credit Reporting Act (15 USC at 1681-1681u) as amended, before we can seek such reports, we must have your written permission to obtain the information. You have the right, upon written request, to a complete and accurate disclosure of the nature and scope of the investigation. You are also entitled to a copy of your Rights under the Fair Credit Reporting Act.
Further, I understand that information about myself will be anonymously (without my name) shared with a prospective mentee(s) and his/her parent(s)/guardian(s) to aid in determining a suitable match. Once a mentor/mentee match is determined, my identity and any other information known about me may be shared with the mentee and parent/guardian to ensure and aid in facilitating a safe and successful match relationship.
