to participate and travel with the school group under the supervision of teacher/supervisor or parent, to winter or summer sport day event, theater play, movie theater during school year 2025/2026.
I/WE HEREBY WAIVE any rights my child/children or we, as parents or guardians, may otherwise have against the school or any of the employees, teachers, administration, volunteers of the St. John Paul II Polish School or the Calgary Catholic School Board or any employees or the Board within the scope of their employment for any damage or injury suffered by our child/children during the said events or trips.
I/WE ACKNOWLEDGE that there are inherent risks associated with these activities and that I/WE or my child(ren) could sustain personal injury through participation in these events and I/WE are hereby accepting to take that risk on behalf of myself or my child(ren).
I/We give permission to publish my/our child(ren) pictures/videos from the St. John Paul II Polish School events on the school's social media. .
