  • Hello, and welcome to the first step in your new tattoo project. Below is a list of details that we need from you in order to help us crelate the best possible tattoo design for you.     ** NOTE**. FILLING OUT A REQUEST FORM DOES NOT MEAN THAT YOU AUTOMATICALLY HAVE AN APPOINTMENT WITH THE ARTIST YOU ARE WANTING TO WORK WITH.

     

    APPOINTMENTS WILL BE SET UP EVENTUALLY, BUT A DEPOSIT IS REQUIRED FOR AN APPOINTMENT WITH ANY OF OUR ARTISTS. THE DETAILS WILL BE LISTED IN THE FOLLOWUP EMAIL TO THIS REQUEST FORM.

We ask that you please understand We are not able to take on every submission and can only book so many people in each month.
