• No waterproof mascara
• No oil based products around the eye area
• No tinting or perming of eyelash extensions
• No pulling, picking, or rubbing of the eyelash extensions
• Wash lashes every morning & night to keep them clean!

Risks & Potential Reactions: 
By signing this consent form and proceeding with your lash appointment, you acknowledge and accept the following risks associated with eyelash extensions:

Possible Allergic Reactions:

• Eyelash extensions may cause allergic reactions at any time, including redness, irritation, itching, swelling, or other symptoms of ocular distress.
• These reactions can occur due to the chemicals in the adhesives or removers used.

Skin and Eye Irritants:

• Adhesives and adhesive removers are known skin and eye irritants.

• You can develop an allergy to the adhesive ingredients at any time, even if you have used them before without issues.

Medical Attention Recommended:

• If you experience any discomfort within the first 24-48 hours after application, contact us immediately to schedule a complimentary removal of the extensions.
• It is strongly advised to consult a doctor if you experience ongoing irritation or other adverse symptoms.

No Refund Policy:

• Refunds are not provided for lash extension services once applied.

Medical Costs Are your Responsibility:

• Any medical attention sought due to reactions or irritations will be at your own expense.

Your safety and satisfaction are our priorities. Please notify us of any concerns promptly.

FILL REQUIREMENTS
To ensure your lash extensions maintain their best appearance, the following policies apply to fills. Ensure your appointment is within the specified time frame and that you meet the lash retention requirement.

Minimum Lash Retention Required:

• You must have at least 50% of your lash extensions remaining to qualify for a fill.
• If less than 50% of your lashes remain, you will be charged for a full set.

Fill Pricing:

• Lash fills are priced uniformly for appointments scheduled within 1–3 weeks of your previous service.
• Appointments exceeding 3 weeks (21 days) or with less than 50% lash retention will require booking a full set. No exceptions can be made.

Full Set Charges Apply if your lashes fall below the 50% threshold or if your appointment exceeds the 21-day window.

MAKE UP REMOVAL FEE

Please come to your appointment free of eye makeup. If you are arriving to your appointment and have any makeup including and not limited to foundation, eyeliner, mascara, eyeshadow etc. You will be charged a fee to remove it. Having makeup or any type of debris on your lashes will affect the retention of your lashes and the glue will not adhere to your lashes. You will be charged for a makeup removal fee of $25.  Removal of eye makeup will cut into your appointment time and result in less lashes applied.

FOREIGN FILLS

Foreign fills are accepted at Admired Radiance with a service fee of $150. However, if we are unable to work over the existing lashes, the client will be charged for a removal and full set price. This is NOT a requirement and you are able to elect NOT to proceed with this decision, but this will result in paying the cancellation fee.

Lash Extension Removal

I understand that professional lash extension removal involves the use of a specialized remover designed to safely dissolve lash extension adhesive. While every precaution is taken to protect the natural lashes, eyes, and surrounding skin, I acknowledge and accept the following:

Temporary sensations such as tingling, mild irritation, stinging, or watery eyes may occur during or after the removal process. In rare cases, redness, swelling, allergic reaction, or discomfort may occur due to sensitivity to the removal product or adhesive residue.
Lash extension removal may result in the loss of natural lashes, especially if the lashes were previously over-processed, improperly applied, or compromised by prior services or at-home removal attempts. Results may vary depending on the condition of my natural lashes, prior lash work performed by another artist, and the age or type of adhesive used.

No Guarantee Clause: I understand that lash extension removal does not guarantee the preservation of all natural lashes and that lash health varies by individual.

 
I confirm that I have not attempted to remove my lash extensions myself using oils, tweezers, steam, or other methods that could weaken or damage my natural lashes prior to this appointment, or I have disclosed such attempts to my technician.

 
I agree to follow all post-removal care instructions provided to me to support lash health and recovery.

 
By signing below, I release and hold harmless the lash artist and business from any liability, claims, or damages arising from the lash extension removal service, provided the service is performed in accordance with professional standards and protocols.

 
I acknowledge that I have read, understood, and consent to this lash extension removal disclosure.

I confirm to the best of my knowledge that the information I have provided is accurate and complete. I have not withheld any information that may be relevant to my treatment and/or the results thereof. I am aware that there are often inherent risks associated with lash extension and lash removal services, and that the services I am about to receive could have unfavorable results including, but not limited to allergic reaction, irritation, burning, redness, scarring, soreness, etc.

I further acknowledge that I have read, understand, and agree to the Booking & Cancellation Policy. By signing below, I agree that I will not hold Admired Radiance, LLC responsible for any unfavorable outcomes or results.

I authorize Admired Radiance, LLC to charge my credit card for any cancellations made less than 48 hours prior to my appointment and for no-show fees. I also consent to having my payment information securely stored on file for future transactions.

Permission to Use Pictures/Videos - I hereby grant Admired Radiance,LLC the full right to take, publish and reproduce photographs/videos of me, my face, my body, both before and after this procedure, for any advertising education, or other purposes whatsoever, including the right to retouch these photographs as deemed necessary. I further expressly assign any copyright in these photographs to my esthetician and her business. I grant my consent to my esthetician to use my image and likeness as contained in these photographs for any advertising or other purposes, along with any comments I may provide. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use. Please use these images with the following:

• Complete ALL prescreen/client waiver form. Please sign the appropriate waiver/consent form prior to coming to your appointment. If you are unable to sign waiver/consent form via online, you can do so in the spa.  However, it is best to come 10min earlier to fill out all proper paperwork. 

• Client only. Please come to your appointment alone. If the client is a minor, an adult is able to escort them until the consultation is completed, then must wait in the lobby of the spa or elsewhere until the service is completed. The spa will only allow entry to clients with appointments.

• A fabric/medical grade mask will be OPTIONAL. Please keep in mind that your mask may get dirty or stained during your service, so use one that you don’t mind compromising. We will not be held responsible to replace any masks if such damage occurs. If you are receiving a facial, you will be asked to remove it.

• No outside food or drinks. We ask that you refrain from bringing any outside food or drinks into the spa, so please eat before coming to your appointment. 

• Pack light. We ask that you please minimize the amount of personal belongings you bring into the spa to limit contact and mitigate exposure in the spa. 

• If you are feeling ill or are experiencing covid-19 symptoms, please reschedule your appoinment.

• Text me (808)387.7754 and wait. When you arrive at the spa, please text me to let me know you are here and wait in your car until I say I’m ready. Please be patient as I may be running behind on a client or still cleaning and sanitizing. 

• Contactless payment. To limit contact, it is highly recommended that payment be contactless I accept (Debit or Credit cards) but if cash payment is preferred, please bring exact change. I will not have cash change readily available during this time.

At any time during our contact with you, if we feel you are not complying with the rules stated above, show signs of illness or are in any way combative we reserve the right to refuse service and will ask you to leave immediately.

By signing this waiver, you understand that by entering a business open to the public, you are susceptible to the risk of exposure to any illness including but not limited to, the Coronavirus also known as COVID-19, and will not hold Admired Radiance, LLC liable for any symptoms of illness following your contact with us and will contact us if you do develop symptoms within 5 days after your visit.
