Athlete Drop Form
1. This form should be completed and submitted to unenroll a student from class. (1 form per student) This is a 30 day drop notice.

2. There will be no refunds for the tuition paid if unenrolling before the last scheduled class of the month.

3. If you are only transferring from one class to another, please call our customer experience team at 859-800-2872 to assist you and find a class option to fit your changed schedule.
