Handicap Index Scores Form

Establish an initial USGA handicap index
If you plan to participate in Competitive HVWGC leagues, either as a league member or substitute, you are required to have established USGA handicap index of 53.9 or less prior to the start of the league.

If you do not have a USGA handicap index, we can establish a beginning handicap for you if you provide a minimum of three 18-hole scores or six 9-hole scores (or a combination) played on USGA/GAM rated courses from the previous year.

Complete this form and we will create an initial handicap index for you.

You can submit this form online by pressing the SUBMIT button or you can print the form and email it to hvwgolfclub@gmail.com.
