SPORTS PHYSICAL

This form is only for 10th - 12th grade students. Incoming FR should have submitted a school physical here before the start of the school year.

Every student must have an up-to-date physical on file before the start of their chosen season. Physical are valid for 1 year from the date of examination. Any student who does not have an up-to-date physical on file before the start of the first day of practice will be withheld from participation until completed. Questions about physicals can be directed via email or Teams chat to Mr. Jason Dillon.

Returning students: Check your school email to see when your sports physical expires. Search for emails from jdillon@cristorey.net
