If you already completed your registration form and just need to upload an updated physical, please do so below.
SPORTS PHYSICAL
Every student must have an up-to-date physical on file before the start of their chosen season. Most physicals are good for 1 year. Any student who does not have and updated physical on file before the start of the first day of practice will be withheld. Questions about physicals can be directed via email or Teams chat to Mr. Jason Dillon.

Please see the sample below to ensure your physical is filled out correctly before submitting it. All highlighted sections MUST BE COMPLETED in order for the physical to be valid.

A request for completion occurs when a signature or other important information is missing from your physical.
