Life Coaching Agreement Form

As the Life Coach, I fully agree:

1.     All sessions will be conducted ethically.

2.     I will use my skills to the best of my ability to help you achieve your desired goals.

3.     I will protect your privacy and respect all of your rights under GDPR. You can contact me at anytime to request your data, change your preferences or request that your data be deleted. GDPR is the European privacy law designed to protect you and give you control of your data.

4.     Notes of the session will be kept securely in accordance with the terms of the Data Protection Act.  Under obligation of professional membership and as part of continued professional development, I may share case history with my peer-support groups.  All information will be anonymous and will not be in breach of professional confidentiality.

As the client, I fully agree:

1.      I will provide honest and accurate information. 

2.      I will set and keep appointments. Late attendance for a session may not guarantee a full session.

3.      Cancellation or re-scheduling of appointments will be made no less than 3 days prior to the appointment.  Cancellation less than 3days to the appointment will result in the full session fee being charged.

4.      All appointments are subject to a deposit of £60 per 1-hour session or £250 for 6 sessions made online.

5.      All sessions will take place online via ZOOM.  I will ensure that I be in a place that is quiet with not disruptions and guarantees my privacy.

6.     I will commit to my treatment and follow through with agreed tasks between sessions.

7.      Contact between sessions will be limited to emails and phone calls.

8.      Any antisocial behaviour will result in the cessation of treatment.

9.      No refund would be offered as a result of unsuccessful therapy.
