*Bona Fide Meal Periods - 3 hours total of meal periods that are each duty-free, uninterrupted, and at least 30 minutes in duration

I attest that every day I am given at least 8 hours for sleep, at least 5 duty-free, uninterrupted, and that I am given 3 duty-free, uninterrupted hours for meals.

If not, for each shift during which I did not receive my duty-free, uninterrupted Sleep and Meal Periods, I will (1) notify my Coordinator within 24 hours of the end of the shift, (2) complete a Sleep and Meal Exception Certification Form and return it to my Coordinator within 72 hours of the end of the shift (3) report any interruptions while clocking out during the the Electronic Visit Verification.
