I agree to do a paid work assessment at the location provided by my Case Manager. The assignment will last for the time specified by my Case Manager.

My Supervisor will be assigned at the worksite or provided to me by my Case Manager.

I understand that I will be paid minimum wage.

I understand that I will perform the following tasks if I work in Goodwill retail: 1. General cleaning 2. Putting away clothes 3. Straightening the sales floor 4. Dressing Room Attendant 5. Sorting and processing of clothing and wares 6. Assisting with donor door

I understand that I will perform the following tasks if I work in Goodwill Industrial Services: 1. Light Assembly 2. Quality Inspections on own product

In both instances, I will need to be able to lift to 50 pounds and stand for long periods of time.

I understand that I will perform the all tasks required at a given worksite (unless my Case Manager has been provided with restrictions from the funder)

I will need to adhere to all policies and procedures, as outlined in the Personnel and Consumer Manual (which were presented to me at Orientation

If I do not follow the rules, I could be disciplined up to and including termination from the worksite or in extreme situations, termination from the program.
