The Program requires supporting documentation to verify your eligibility.
Most Recent Federal Tax Return Form 1040: Please provide pages 1 and 2 of your most recent federal tax return Form 1040 which shows gross income. Each member of the household that filed a federal tax return must provide these documents.
1. If current income is significantly different than income reported on the most recent tax return, please contact us for monthly income application guidelines and document requirements.
2. If you or a household member do not file federal tax returns, please supply income verification documents you do have such as a social security award letter, pension award letter, etc.
Proof of Homeownership: Acceptable documents include: property tax bill from your county, warranty deed from your closing paperwork, or approved homestead exemption form.
NOTE: If you are a renter, you and your landlord will complete the simple Landlord Consent Form.
APPLICANTS SHOULD BLACK OUT ALL SOCIAL SECURITY NUMBERS AND BANK ACCOUNT NUMBERS LISTED ON THEIR SUPPORTING DOCUMENTATION.
