Session Questionnaire

New and returning clients, welcome. You’ve read my Etiquette form, know your interests, have session dates in mind and are read to schedule an appointment. You may book your appointment by submitting this session questionnaire.

Vetting/Screening Protocol 

Before I will see you, I require:

Two references from other professional providers that you have seen. If you have never seen a professional before, you will need to screen through the protocol below.

No references:

Clear picture of you, plus one of your driver's license, with all information clearly visible. I will verify this when we meet. We will need to meet for a compensated coffee meeting in the South Bay, before booking. Once we meet, you will be required to pay for the entire session up front. I accept Venmo, or you may provide your credit card information via phone. You may also provide cash during our initial meeting. I appreciate your understanding my need for taking extra precautionary measures to ensure my safety.

BDSM Risk Acknowledgement

Engaging in BDSM activities comes with many risk factors which can result in intentional or unintentional, permanent or temporary, bruising, welts, skin abrasions, cuts, burns, blisters, bleeding, broken bones, concussion, heart palpitations, fainting, heart attack, blood clots and even death (and many other conditions too numerous to mention). By checking the box below and by submitting this form you acknowledge that you understand the risks and are engaging in BDSM activities with the understanding of the possibility of the above. You hold harmless all participants that will be engaging with you in any and all BDSM activities.

Tardiness and Cancellations

If you are late, I expect to be compensated for our scheduled time in its entirety. I may provide a 10 minute grace period at my discretion. You must reschedule at least 48 hours in advance in order to transfer your deposit to your new appointment. The new appointment must be within one month of your current one or your deposit will be forfeited. Any cancellations or requests to reschedule an appointment with less than 48 hours notice will result in full forfeiture of your deposit. If you cancel with less than 24 hours notice, I will require 50% of session tribute for you to remain in good standing. Should you need to cancel your appointment completely, please call me as soon as possible to let me know, however you will be forfeiting your deposit.

My Limits

Anything ending in "job", brow, or ruby showers, worship of my bare breasts, bottom, or genitals, Switching/Subbing. Any activity that is directly sexual, illegal, unsafe, non-consensual, or that has not been negotiated prior to beginning of session. As a reminder, our mutual discretion, safety and privacy are of the utmost importance to me.

Cross Dressing Specifics

HEALTH; participating in sessions involving BDSM activities can put stress on the body. Please disclose any and all medical conditions, so Mistress may determine what activities should be modified or excluded. Heart conditions, chest pain, faining spells, dizziness, or high blood pressure. All surgical implants, including pacemakers. Diabetes, asthma, respiratory and breathing difficulties must be disclosed. High or low blood surag problems. Claustrophobia, muscular and skeletal issues, injuries, broken bones, particularly neck and knee issues. Allergies to latex, rubber, talc, and nonoxynol-9. Contact lens wearing, cataracts, and glaucoma. Any and all mental health issues that you have suffered with, been diagnosed with, taken medication for, including addictions. It is your resposibility to disclose any of the above or anything else that may not be listed.

There is no guarantee that Mistress will be available on the date and time you selected. Please provide as many options as possible, to assure that Mistress may be able to accomodate you. Once this form is submitted, you will receive a confirmation email in which Mistress will let you know which date and time work best with her schedule.

INFORMED CONSENT STATEMENT; I am requesting and at my own risk participating in a BDSM session. I understand activities within the Session will be conducted by mutual consent. I accept sole responsibility to; (a) decide what activities I wish to participate in during the Session and decide what my boundaries are. (b) communicate these decisions clearly before the start of the Session. (c) agree to an appropriate safe word or other signal to request an activity stop. (d) signal immediately, any unexpected fatigue, shortness of breath, chest discomfort, nausea, pain or discomfort. I understand I can discontinue participation at any time during the Session, without prejudice against me, beyond fees or tribute for the Session. LIMITS OF LIABILITY; I understand that any activity undertaken during the Session are voluntary and are at my own risk. I agree to indemnify and hold harmless all participants in the Session from all claims of liability, injury, death, property loss or damage, attorney’s fees and costs and I agree to reimburse them for any such claims; of same, they may have. AGREEMENT; by checking the box and submitting this session application, I acknowledge I have read, understood and agree to the contents of this form and I confirm that all information given by me, is correct to the best of my knowledge. I understand there is no sex involved in Mistress Sterling's sessions, I am a consenting adult over the age of 21 and I am not a law enforcement officer or federal law enforcement agent.
