I, the client, here by agrees to the following terms in order to make an appointment or to have anything drawn for a tattoo to be done by any artist at Hive Tattoo Studio.

1.  I am at least 18 years of age or will be at the time the tattoo will be done. (Government issued I.D. required to get a tattoo.)

2. I will pay a deposit, which is NOT REFUNDABLE. I understand that if I keep my appointment, my deposit will go toward the balance owed on the tattoo I have picked. If I have something drawn, and do not set up an appointment within one month of completion of the drawing, I will lose my deposit and be required to pay a new deposit to set up an appointment.

NO APPOINTMENT CAN BE MADE WITHOUT A DEPOSIT.  ALL DEPOSITS ARE NOT REFUNDABLE.

3. I understand that appointment dates may only be changed with 7 days notice. Anything less than 7 days and I forfeit my deposit and be required to pay a new deposit.

4. I understand that the tattoo I have picked and made an appointment for is what I am getting.

ANY LAST-MINUTE CHANGES I WISH TO MAKE TO MY TATTOO, WITHOUT AT LEAST 14 DAYS NOTICE, MAY RESULT IN FORFIET OF MY DEPOSIT AND I MAY BE REQUIRED TO PAY ANOTHER DEPOSIT AND MAKE A NEW APPOINTMENT FOR A LATER DATE.

5. I will be required to be on time for my appointment. If I am more than 15 minutes late, I will forfeit my deposit and be required to pay a new deposit and set a new appointment.

6.  If I am a legal guardian of a minor child setting an appointment to get a tattoo on or after their 18th birthday, I agree to the terms and conditions listed above.

7. Any "addtional" money -in addition to the deposit that is put down towards a tattoo- is refundable.

8. If my artist needs to reschedule for ANY REASON, I understand that my deposit is still nonrefundable. I have the option to reschedule at their next available day that they offer.
