  •      We are excited to begin treatment of your child. Please note that it is expected that a parent or guardian will attend all visits with their minor child. We understand that at times it may be more therapeutic to have a parent or guardian step out of the appointment for at least a portion of the time. We do however want a parent or guardian present or available to discuss care for sensitive issues such as medication management, scheduling etc.
     
         Please note that the patient’s care will be discussed in its entirety during the course of the visit. If as the guardian you would like to have input into the care plan or discuss treatment options you must attend the visit. We can provide a copy of the provider notes for the visit however providers will not be able to recap the visit in any other way or at any time other than during the visit. Providers will not be able to call or email to recap a visit with a parent or guardian. Parents or guardians are expected to attend the visit in order to have the full knowledge of the patient’s care. This will provide ample opportunity to give input or express concerns.
     
         If as a parent or a guardian you do not wish to attend your child’s visits you may sign the release allowing the provider to meet with your child without a parent or guardian present.

  • I understand the expectations and policy of Phoenix Mental Health and Wellness in regards to the care of my minor child.

  •      I hereby give consent for Phoenix Mental Health and Wellness to provide psychiatric services for my minor child without my presence at the appointments. I consent to allow my child and the provider to establish a treatment plan including prescribing medications as needed for the treatment of my minor child.
