This agreement entered into this date by and between DOEFS, in the City of BROWNSVILLE, TN., and the applicant, hereinafter the Adopter, for the adoption of the above-identified pet. Please initial at the end to indicate agreement
1.The adopter agrees to pay DOEFS the adoption fees upfront before taking possession of the pet(s).
2. The adopter agrees to provide the pet with a good, safe home with proper care and attention. When the pet is outside it shall be provided protection from the elements by a dry, insulated, draft-free house, and protection from predators. The pet shall be provided with shade from the sun and adequate water and food. It is further agreed that the pet not live on a chain or any similar restraining device. For any pet living outdoors, the shelter must have 3 full sides, a roof, and floor. The shelter must be large enough for the animal to stand up/turn completely around inside.
3. The adopter agrees this pet will be examined by a licensed veterinarian at least once a year and will receive required vaccinations and treatments to ensure good health including monthly heartworm/disease/pests preventatives.
4. In the event DOEFS discovers the inhumane treatment of this pet OR this pet is living chained OR for the inability to collect on a bad check, it is agreed that DOEFS has the legal right to take immediate possession of the pet. If the pet is required to be spayed or neutered and has not been at the time of adoption, the adopter agrees to have them spayed or neutered within 30 days of adoption at their expense. 5. In the event this pet becomes injured or ill, the adopter agrees to seek immediate and proper medical attention from a licensed veterinarian. If euthanasia becomes necessary, it will be done by a licensed veterinarian. The death of the animal should be reported to DOEFS.
6. The adopter agrees not to use this pet for experimentation, breeding or for resale. If adopter wishes to re-home the animal, they must return the pet to our care.
7. In accordance with the TN State laws, the adopted animal will be responsibly confined to the owner's property and not allowed to roam. If Adopter is from another state those laws will be researched and adhered to.
8. The adopter understands that if for any reason they decide not to keep this pet, it will be returned to DOEFS immediately, and further understands that the adoption fee is non-refundable under any circumstances.
9. DOEFS makes no warranties regarding a pet's age, health, breed or temperament. It is agreed and understood by the adopter that while every effort is made to provide accurate history and assessment of a pet, DOEFS does NOT warranty a pet regarding age, medical status, breed, behavior or disposition. It is recommended that you take your new pet to your vet within 7 days of adoption for a complete examination. The adopter agrees to indemnify and hold harmless DOEFS and its representatives against any and all claims for any injury or property damage to the adopter or anyone else.
10. The adopter understands that the DOEFS reserves the right to conduct home visits before and/or after the adoption, with or without notice for the remainder of the pet's life.
11. The adopter agrees to notify DOEFS if the animal becomes lost, deceased, or stolen.
