As per Age Qualifications and Dispensation Procedures in the 2021-2022 Regulations and Playing Conditions, a club may apply for dispensation for any player who does not reach the age qualifications as outlined.

Please note a support letter from CFN Director of Coaching, support letter from minimum a qualified level 1 coach (club coach, rep coach, etc) and approval via signature on this form is from one parent/guardian i srequired. You will need all this to complete the form.
