I understand that before my child is enrolled in the school, I, and any other guardian or parent who assumes financial responsbility for the child, must sign an Enrollment Contract and a Safety Contract, as well as provide an Immunization record for my child’s file.
I also understand that I must complete additional paperwork for my child’s file. This includes but is not limited to a signed and dated Parent Handbook Acknowledgement form, Allergy & Anaphylaxis forms (as needed), and Health Authorization forms.
I further understand that my child will be required to wear a mask during school hours. Failure to comply with this policy may result in dismissal from the program.
In addition, all adults, regardless of vaccination status, visiting the school must wear a mask while indoors or in proximity to children.
