COVID-19 Waiver and Questionnaire
With the ongoing COVID-19 concerns across the region, Fabian Joseph Coaching is taking extra precautions based on recommendations from the Government to ensure the safety of our athletes and families.
PRECAUTIONS AND MEASURES
All precautions and measures set by the rink must be adhered to by Fabian Joseph and any athletes or family members attending sessions. These will vary depending on the rink and the current COVID level in the area.

Social distancing will be in place whenever possible on and off the ice.

Fabian Joseph will adhere to high levels of health and safety, using hand sanitizer and/or washing his hands as needed. Players will be reminded to wash their hands and/or use hand sanitizer.
POLICY FOR COVID-19
If you or your family members have traveled outside of your province or Canada in the past 14 days or have been in close contact with a confirmed or suspected COVID-19 case and are now experiencing symptoms, we ask that you refrain from attending your scheduled session.

In the event of any symptoms that arise during your athletes training time please alert me immediately so I can contact the Health Department and follow their guidance.

If any athlete is showing signs of illness they will be sent home immediately.
QUESTIONNAIRE
Have you or a member of your household arrived from travel abroad(outside of Canada) or outside of your province within the last 14 days?
Have you or a member of your household experienced any symptoms associated with COVID-19, such as dry cough, fever, fatigue/extreme tiredness, difficulty breathing or pneumonia?
Have you or a member of your household been in close contact with anyone know or suspected to have contracted COVID-19 in the last two weeks?
