With my participation, I have consented to my child’s or ward’s participation in the activity, and have agreed individually and on behalf of the child or ward and that I fully understand its content. I am aware that this is a release of liability and is a contract made in consideration of my participation in the Activity.
With my participation, I hereby grant Capital Village Schools the right and permission to use photographs and/or video recordings of my child, myself, and my guests in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me. Furthermore, I agree that all such media are and shall remain the property of Capital Village Schools.
